Proper medication management is crucial in preventing medication errors, improving client outcomes, and minimizing the risks associated with medication use.
Most seniors often take a variety of medications, increasing the risk of drug interactions and side effects. One key component to managing medications is that healthcare providers should regularly review them to confirm they are essential and appropriate.
Caregivers should be familiar with the client’s correct medication, dosage, and instructions provided by the healthcare providers; this will ensure that the patient is taking the medication correctly (correct dose, time, and method). Frequently checking the effectiveness of the medications and watching for side effects or interactions with other medications is vitally important.
Caregivers should educate the client about the medications, names of drugs, how and when to take them, possible side effects, and what to do if a dose is missed. Regularly review and update the client’s medication list to ensure it is current while increasing the client’s knowledge and assisting during the transition of care.
Helping clients stick to their medication plan through reminders, pill organizers, and follow-up is always beneficial. Alarmed pill dispensers, smartphone apps, or phone calls can remind seniors when to take their medication.
Medication management for seniors is particularly important due to the unique challenges older adults face due to multiple chronic conditions, a higher likelihood of taking multiple medications, and cognitive decline.
