One of our highly experienced care managers will meet your loved one at the facility upon discharge, ensure safe transition to home or facility and assist the family with implementing post-discharge instructions. This service provides up to 10 total hours of care manager time, including travel time and mileage.

• Have you received notice that a loved one is being discharged from a hospital or a facility in the next 48 hours? One of our highly experienced care managers will meet your loved one at the facility upon discharge, ensure safe transition to home, provide a full clinical assessment and assist the family with implementing post-discharge instructions.
  • Includes coordinating needs for any additional support such as home care, skilled services, hospice, durable medical equipment and scheduling any follow-up medical appointments.

This service provides a monthly visit with the patient, all needed calls and communications with the patient and family during the month, as well as coordination of appointments, resources, and calls with physician and medical support team members.

One of our highly experienced care managers will meet with the patient and family (virtually or in-person) to gather all necessary information to complete a full clinical assessment. This includes performing all needed tests, as well as researching all appropriate options and resources. A complete care plan with recommendations will be created and reviewed with the patient and family.
