Same Day Discharge Package

A discharge from a facility can happen suddenly. When you need help right away, our Same Day Discharge Package is security for you and your family. One of our highly experienced care managers will meet your loved one at the facility upon discharge, ensure safe transition to home or facility and assist the family with implementing post-discharge instructions. This service provides up to 10 total hours of care manager time, including travel time and mileage.

Transition Package
  • Are you a current patient, or do you need help with transitioning a loved one? The Option Group will research all candidate facilities, assess them based on patient needs, create a personalized transition plan and review with the patient and family.
    • This service is for patients who have previously received a full clinical assessment by The Option Group.
Planned Discharge Package
  • Have you received notice that a loved one is being discharged from a hospital or a facility in the next 48 hours? One of our highly experienced care managers will meet your loved one at the facility upon discharge, ensure safe transition to home, provide a full clinical assessment and assist the family with implementing post-discharge instructions.
    • Includes coordinating needs for any additional support such as home care, skilled services, hospice, durable medical equipment and scheduling any follow-up medical appointments.
    • The planned discharge package includes up to 10 total hours of care manager time, including travel time and mileage!
New Patient Assessment and Transition Package
  • This popular package provides a full clinical assessment from one of our expert care managers, along with researching, assessing and vetting of all candidate facilities based on patient needs. A comprehensive transition plan is created and reviewed with the patient and family.
    • The assessment and transition package provides up to 10 total hours of care manager time, including travel time and mileage!
Safe-At-Home PLUS Subscription
  • This service includes everything in our Safe-At-Home subscription and provides up to 2 monthly visits with the patient.
    • The 2 monthly visits can be 1:1, virtual visits (or a combination), or attending a scheduled appointment with the patient (such as a Dr’s appointment, dentist, legal, financial, etc.). When our care manager attends a physician visit, we receive important medical information first-hand and can ask key follow-up questions, ensuring patient advocacy for your loved one.
    • Care manager travel time and mileage are included!
Safe-At-Home Subscription
  • Make sure your loved one is always Safe-At-Home. This service provides a monthly visit with the patient, all needed calls and communications with the patient and family during the month, as well as coordination of appointments, resources, and calls with physician and medical support team members. 
    • One monthly visit can be a 1:1 visit, a virtual visit, or attending a scheduled appointment with the patient (such as a Dr’s appointment, dentist, legal, financial, etc.). When our care manager attends a physician visit, we receive important medical information first-hand and can ask key follow up questions, ensuring patient advocacy for your loved one.
    • Care manager travel time and mileage are included!
Initial Welcome Package

One of our highly experienced care managers will meet with the patient and family (virtually or in-person) to gather all necessary information to complete a full clinical assessment. This includes performing all needed tests, as well as researching all appropriate options and resources. A complete care plan with recommendations will be created and reviewed with the patient and family.
